The enrolment form
For government-funded places only. You need it before the course opens.
What you need
Your National Insurance number, passport details, your employer’s details.
Changing it
Edit it any time until the training provider has checked it. After that, submit a new form.
Sensitive questions
Ethnicity, health and convictions: only you, the training provider and trainrs’ owner see them.
